Presumably abl. Ḫišḫuraz, rather than Ḫišḫura=za.
The sentence “the snake of the head of …” appears to be a standard sentence in snake oracles (cf. several examples in e.g. KBo 23.117, IBot 1.33, KBo 53.107+, KBo 63.68, KUB 49.1).
